This notice provides guidance on how patients may request access to their medical records, contact the appropriate licensing or disciplinary authority, and file a consumer complaint.
1. Requesting Your Medical Records

- Current contact information
- A description of the specific records or date range you are requesting

3. Filing a Consumer Complaint
Patients who wish to file a complaint against a licensee may do so by contacting the Texas Board of Chiropractic Examiners using the contact information provided above.
Public Interest Information
Under the Chiropractic Act of Texas, the Texas Board of Chiropractic Examiners is required to establish methods for notifying consumers and service recipients of the Board’s name, mailing address, and telephone number for the purpose of directing complaints to the Board.
Patients may also refer to the Texas Attorney General’s consumer information resources for additional guidance regarding health information privacy rights and consumer complaint procedures, as outlined in Texas Health and Safety Code Section 181.103.
